If you are interested in enrolling your child, below you will find forms which are required to be completed prior to your child's attendance. You may either print these forms and begin completing them in the comfort of your home, or, if you prefer, we have them available in our office and they can be completed in person. It is not mandatory nor required to have the forms completed before touring the school or making an appointment for enrollment.
